Home Care Coordination Can Help.
Home Care Coordination is a private, non-profit organization committed to helping elderly and disabled adults age 18 and older live independently in their own homes. If you are having difficulty taking care of yourself or a loved one, Home Care Coordination can offer an economical alternative to expensive institutionalization.

We can relieve anxiety and concern by helping clarify problems, identify care needs, develop and choose options, and obtain the appropriate community resources at the lowest cost.

Our professional staff consists of highly-trained nurses and social workers. The staff members will meet the elderly or disabled person and family members in the home to discuss care needs. We then organize an individualized plan of care. Follow-up visits are provided to oversee the situation in the home and to ensure that the care is adequately meeting the person's needs
