Helping people on the path to recovery from addiction

The Horizons Program is designed to help persons with developmental disabilities in recovery from addictions.

This program helps those in dealing with alcohol, drugs, and gambling addictions, as well as, pornagraphy, compulsive spending, and other dependencies.

The program provides recovery groups and helps participants develop better coping skills to deal with difficulties. The many activities provided by the program also help people develop a support system and a social life free of addictive behaviors.

The Horizons Program is part of Community Care Management Corporation, a non-profit agency serving Mendocino and Lake Counties. For over 20 years Community Care has provided support to enable people with disabilities to remain in the community, in their own homes, at the highest possible level of independence.

Who can obtain services?
To qualify for Horizons Program services you must:
1) Be a consumer of services through the Redwood Coast Regional Center.
2) Be referred, by request, through your case manager at Redwood Coast Regional Center.
3) Live in the greater Ukiah or Willits area.
4) Have substance abuse or process addiction issues, and have barriers to treatment within the current alcohol and other drug treatment programs.

Horizons Program Activities
The Horizons Program provides and facilitates activities that are positive and life affirming to replace the negative, life threatening activities. Activities are sometimes spiritual in nature, such as meditation or attending twelve step oriented recovery groups which help build new healthy support systems. Clients have opportunities to participate in Native American drumming, a sweat lodge, camping, creative arts, and various healthy and positive social activities to develop self-awareness and self-esteem. Most importantly, help reduce feelings of isolation, a root cause of most addictions.

Participants attend activities at the local health club, with personal trainer assistance if needed, to enhance their overall health and well-being. This approach also fosters growth in self-esteem and augments the individualís participation in the community as a valued member.

Group and one-on-one counseling are provided, along with advocacy in legal, medical, and mental health areas.

Voluntary Contributions:
Your voluntary contributions are greatly appreciated.
   Funds are used to pay for much needed client services that our grants don't cover. In most cases, since much of our administration expenses are paid by other funding sources, your contribution can be used almost entirely to pay for client services. Many contributions are in memory of a loved one.
   Checks can be made out to CCMC. If you wish, you can make a notation to indicate which program you would like your gift to go towards.
