Services for elderly and younger adults with disabilities


Linkages serves adults with disabilities who need help to manage at home.

NOTE: THE LINKAGES PROGRAM WAS ELIMINATED FROM THE STATE BUDGET ON OCTOBER 1, 2009. IT IS UNKNOWN IF THIS PROGRAM WILL BE RESTORED.

To Qualify for Linkages You Must:
1) Be at least 18 years of age; and
2) Have a disability which requires some assistance to live independently; and
3) Live in the Linkages service area.

(Note: Linkages cannot serve persons who are eligible for Regional Center, Mental Health, or Department of Rehabilitation services.)

Coverage Area:
Lake County:
Clearlake, Clearlake Oaks, Clearlake Park, Glenhaven, Lower Lake, Middletown, and Lucerne.
Mendocino County:
Ukiah, Willits, Hopland, Potter Valley, Redwood Valley, and the Fort Bragg / Mendocino area.

Program Description:
Linkages' most basic goal is to help persons with disabilities live an independent life of dignity at home (vs. institutionalization).

Linkages provides a caring and qualified Care Manager to meet with you to learn your needs for independent home living. Working together, you'll develop a plan to meet those needs by drawing on existing community resources or, in some cases, through outright purchase by the Linkages Program. Depending on your financial ability, you may pay for some of the costs of your services.

Your Linkages Care Manager then arranges for the assistance and will check regularly to make sure that the services continue to meet your needs. Should your needs change, your Care Manager will alter your plan and services to meet those needs.

Linkages Services are Important Because:
Most people are happier, and their lives are more fulfilled and connected when they are living at home rather than in an institution. Linkages services can often make it possible for people to remain at home. An added benefit of Linkages services is that it is much less expensive for both the individual and for the taxpayer to help a person to live at home than to pay the cost of an institution.

Some of the Home Care Services that Linkages May Help You Obtain Are:
- Meal preparation or home delivered meals
- Emergency alert devices
- Help with government or insurance paperwork
- Physical and occupational therapy
- Transportation
- Protective supervision
- Nursing care
- Support groups
- Respite
- Legal advice
- Personal care (bathing, dressing, grooming, etc.)
- Home repairs and safety equipment (smoke alarms, ramps, grab bars, etc.)
- A friendly visitor (for the socially-isolated)
- Psychological counseling
- Housecleaning
- Shopping
- Medical monitoring
- Medication setups
- Low-cost housing
- Other assistance as needed

Linkages is not a crisis intervention service for medical, mental health, or other types of emergencies. Nor is Linkages able to provide immediate services prior to enrollment.
